Mobile Health is the occupational health company contracted by PPL to conduct all required medical screenings for New York CDPAP Personal Assistants. Our role is to ensure that all new and transitioning PAs meet the New York Department of Health regulations to start or continue their employment as a caregiver.

A new Personal Assistant (PA) is someone who has never previously worked as a PA in New York State’s Consumer Directed Personal Assistance Program (CDPAP) and is being hired for the first time to provide home care services to a consumer.

Please note that Mobile Health CANNOT help you register with PPL, and CANNOT schedule your medical screening until after you’ve registered with and received clearance from PPL.
What does the medical screening for NEW Personal Assistants (PAs) include?
For all NEW Personal Assistants, the medical screening will include:
- A physical exam ensuring PAs are healthy enough to serve.
- Certification of immunity (called a titer, which is a blood draw) for Measles and Rubella, or vaccination for Measles, Mumps, and Rubella (MMR).
- TB test through a PPD skin test or QuantiFERON blood test, with a chest x-ray if results are positive.
- An annual TB risk assessment for negative cases.
- Proof of a flu shot or a signed declination form acknowledging the requirement to wear surgical or procedure masks in areas where consumers may be present during the influenza season.
- Drug attestation ensuring no drugs of abuse are being used by the PAs.
Please note that Mobile Health CANNOT schedule your medical screening until after you’ve registered with and received clearance from PPL. Mobile Health will contact you by email, text, and/or phone when you can schedule your appointment.
How is an EXISTING Personal Assistant (PA) defined by PPL and the NY CDPAP?
An existing PA is a Personal Assistant who is currently working or has previously worked in New York State’s Consumer Directed Personal Assistance Program and has already transitioned to PPL.
How do I schedule my appointment with Mobile Health as an EXISTING Personal Assistant?
FOR EXISTING PERSONAL ASSISTANTS ONLY
In early April and AFTER your transition to PPL is processed, you can schedule your appointment. Come back in early April to schedule your appointment with Mobile Health.
Please note that Mobile Health CANNOT schedule transitioning Personal Assistants until early April.
What does the medical screening for EXISTING Personal Assistants (PAs) include?
For all EXISTING Personal Assistants, the medical screening may include:
- A physical exam ensuring PAs are healthy enough to serve.

Do I need to complete a new medical screening with Mobile Health if I'm an EXISTING Personal Assistant (PA)?
It depends on when you last completed a medical screening. The documentation you provide will determine which procedures are required at the appointment. Please note that all existing PAs must have valid medical documentation on file by October 1, 2025.
If I'm an EXISTING Personal Assistant, will Mobile Health test for things that weren't tested in my previous medical screening?
Possibly. Requirements may have changed since your last screening. Mobile Health will ensure that your screening meets all current New York Department of Health requirements for CDPAP Personal Assistants.
If I'm an EXISTING Personal Assistant, can I submit my existing medical records instead of doing a new medical screening?
You may submit your existing medical records to PPL. PPL will pass your documentation to Mobile Health, and Mobile Health will determine if your records meet current requirements, or if additional testing is needed.
What is Mobile Health's role with PPL in the NY CDPAP?
Mobile Health is an occupational health company contracted by PPL to conduct all required medical screenings for NY CDPAP Personal Assistants (PAs). They ensure that PAs meet all health requirements set by New York Department of Health before beginning employment.
Is there a deadline for completing my medical screening with Mobile Health?
For NEW Personal Assistants, once cleared by PPL, medical screenings should be scheduled and completed ASAP.
For EXISTING Personal Assistants, valid medical screening documentation must be on file by October 1, 2025.
Can I go to another health provider to complete my medical screening?
If you complete your medical screening with Mobile Health, PPL will pay for it AND Mobile Health will handle all of your compliance documentation.
If you go through another provider, you will have to pay for it yourself and upload all documentation to PPL. This could delay your medical clearance and eligibility to work as a PPL Personal Assistant.
How soon can I get an appointment with Mobile Health?
Due to the high volume of screenings required by PPL for the NY CDPAP, appointment availability may vary. For the best availability, please schedule your appointment ASAP.
Where are there Mobile Health locations in New York?
Mobile Health has 40+ locations throughout New York City and 240+ across the state. You can select your location when scheduling.Â

Can I change my appointment?
Yes, you can change your appointment by logging into our Patient Portal. You can also call Mobile Health at 646-680-0450 to change your appointment. Changes must be made at least 24 hours before your scheduled appointment, and are based upon availability.

NEW Personal Assistants should reschedule and complete their medical screeing ASAP if they still want to be a PA.
EXISTING Personal Assistants in the NY CDPAP must reschedule and have valid medical documentation on file by October 1, 2025.
What happens if I don't schedule or complete the medical screening?
You cannot work as a PPL Personal Assistant in the NY CDPAP.
Why do I need a medical screening?
Medical screenings ensure you’re physically able to perform the duties of a Personal Assistant (PA) and comply with New York Department of Health regulations. This screening protects both you and the consumer you’ll be caring for.
Is there a cost for the medical screening?
If you do your medical screening with Mobile Health, PPL pays for it, so it is free for you.Â
If you go to another provider for your medical screening, you will have to pay for it.
What do I need to bring to my appointment?
You only need to bring two things to your medical screening:
1. A government-issued photo ID, such as a state ID, driver’s license, or passport.
2. Your Mobile Health Confirmation email (can be printed or shown on your cell phone).
Do I need to fast for the medical screening?
No, fasting is not required for the PPL NY CDPAP medical screening.
Do I need my immunization (vaccination) records?
If you have them, please upload them to PPL when asked. This will speed up the process. If you don’t have them, Mobile Health will perform the required tests to check for existing immunity, which are called titers.
If I have documented proof of immunizations (vaccines) to I need to re-immunize (get the same vaccines again)?
No, if you have valid documentation of your immunizations, you will not need to be vaccinated again. Mobile Health will verify your documentation.
What immunizations (vaccines) are required?
- Measles and Rubella: if no immunity present, the MMR vaccine will be offered.
- Flu vaccine: This is optional, but a mask must be worn during flu season if declined.
Can I decline a vaccine?
Some vaccines may be declined for medical or religious reasons, but you’ll need to provide exemption paperwork. Please call PPL at 1-833-247-5346 for details on the exemption process.
How long will my results take?
Most results take 3-5 business days. Mobile Health will send them directly to PPL.
What if a health concern is found during my screening?
Your clinician will explain any findings and may recommend follow-up with your primary care provider. PPL will contact you if there are any concerns that might affect your eligibility to become a Personal Assistant.
Do I need to do anything after completing the screening?
Sometimes, additional procedures are needed after an initial medical screening. If that is the case, Mobile Health will contact you. If no additional procedures are needed, you will learn from PPL whether or not you are cleared to work as a Personal Assistant.
How often do I need to complete a medical screening?
Medical screenings are typically required upon initial hiring and may be required annually or if there are significant changes to your health status or job duties.
Who do I contact if I have questions about my medical screening?

Will Mobile Health notify me about my results?
No. Mobile Health will send your results directly to PPL. However, if there are any immediate concerns, they will be addressed during your appointment.
